Engraving Instructions
If you would like to have the tag engraved, please complete Lines 1 through 4, as indicated below.

• Small: A maximum of two lines of engraving will fit on the Small tag (for example, dog's name and 1 phone number OR 2 phone numbers).
• Medium: Up to 3 lines of engraving will fit on the Medium tag (for example, dog's name, street address and 1 phone number OR dog's name and 2 phone numbers).
• Large: Up to 4 lines of engraving will fit on the Large tag only (for example, dog's name, street address, city, and 1 phone number OR dog's name and 3 phone numbers).
